Anesthesia Privileges

Requirements

  1. A dentist licensed in Texas shall obtain an anesthesia permit issued by the TSBDE for the following procedures used for the purpose of performing dentistry:
    1. Nitrous Oxide/Oxygen Inhalation Sedation.  This permit is required if the licensee plans to use nitrous oxide/oxygen inhalation sedation in combination with any other type of sedation.
    2. Level 1:  Minimal Sedation
    3. Level 2:  Moderate Enteral Sedation
    4. Level 3:  Moderate Parenteral Sedation
    5. Level 4:  Deep Sedation or General Anesthesia
  2. An applicant for a sedation/anesthesia permit must be licensed and should be in good standing with the TSBDE.  “Good Standing” means that the dentist’s license is not suspended, whether or not the suspension is probated.  Applications from licensees who are not in good standing may not be approved.

Application Process

The following is required to be mailed to the TSBDE:

  1. Anesthesia Permit Application.
  2. Pay required fee. 
  3. Copy of current Basic Life Support CPR card and PALS or ACLS certification.
  4. Proof of completion of a qualifying course and case management experience.  Training and case management experience is found on each application.
  5. National Practitioner Data Bank (NPDB) Self-Query.  All applicants are required to complete a self-query of the NPDB.  The report results must remain in the original sealed envelope and be attached to your application to the TSBDE.   NPDB self-query reports are valid for 30 days.  NPDB charges a minimal fee to furnish this information. If you are applying for more than one level of sedation, in one mailing, you may submit one NPDB Self-Query Report which will apply towards all levels of sedation.  If you are applying for sedation privileges on separate dates you must submit a NPDB Self-Query report with each anesthesia permit application.  You can contact the NPDB at Post Office Box 10832, Chantilly, VA 22021, telephone number (800) 767-6732, or website  http://www.npdb.hrsa.gov/pract/howToGetStarted.jsp
  6. American Association of Dental Boards (AADB) Clearinghouse Self-Query.  Applicants are required to complete a self-query of the AADB Clearinghouse.  The report results must remain in the original sealed envelope and be attached to your application to the TSBDE.   AADB charges a minimal fee to furnish this information.  If you are applying for more than one level of sedation, in one mailing, you may submit one AADB Self-Query Report which will apply towards all levels of sedation.  If you are applying for sedation privileges on separate dates you must submit a AADB Self-Query report with each anesthesia permit application. The AADB self-query form can be downloaded from the AADB website at http://dentalboards.org/clearinghouse/.  The AADB self-query form should be mailed to:  American Association of Dental Boards, 211 East Chicago Avenue, Suite 760, Chicago, IL 60611.  

Additional Certificates

It is not necessary to submit another anesthesia permit application to obtain multiple copies of your license for display in satellite offices. You may mail to the TSBDE the Duplicate Annual Registration Certificate Request Form along with the required fee.

Continuing Education Requirements

A dentist seeking to renew an anesthesia permit must complete the following hours of CE once every two years on the administration of or medical emergencies associated with the permitted level of sedation.  CE requirements shall be in addition to any additional courses required for licensure.  ACLS and PALS courses may be used to fulfill the CE requirement when not being taken for the renewal of the anesthesia permit.

Level 1: Minimal Sedation – Six (6) Hours

Level 2 and 3: Moderate Sedation  - Eight (8) Hours

Level 4:  Deep Sedation or General Anesthesia – 12 hours

CPR Requirements

Nitrous Oxide/Oxygen Inhalation Sedation

Basic Life Support only

Level 1: Minimal Sedation
  • Basic Life Support -  Required
  • ACLS or PALS - Not Required.
Level 2, Level 3 and Level 4 Sedation
  • Basic Life Support – Required
  • ACLS or PALS Required – If treating adults and children 13 years of age or older.
  • PALS – Required – If treating children 12 years of age of younger.

Portability Privilege

A Texas licensed dentist who hold a Level 3 or Level 4 Anesthesia Permit may apply for portability privileges.  Requirements are outlined on the Portablity Application.  

Applicants for Level 3 portability must attach:
  1. Anesthesia ​Portability Application.
  2. Pay required fee.
  3. ​Copy of current Basic ​Life Support CPR card and PALS or ACLS certification​.
  4. ​Proof of administration of no less than 30 cases of personal administration of Level 3 sedation on patients in a primary or satellite practice location within the six (6) month period preceding the application for portability, but following the issuance of the sedation permit. Acceptable documentation shall include, but not be limited to, patient records demonstrating the applicant's anesthetic technique, as well as provision of services by the applicant within the minimum standard of care.
  5. Copy of any documentation you provide to the patient (pre- and post-op instructions and contact information).
  6.   2. Pay required fee. 
  3. ​Copy of current Basic ​Life Support CPR card​ and PALS or ACLS certification​.
  4. ​Copy of program completion certificate used to qualify for the permit to administer Level 4 sedation.

  1. Review TSBDE Rules 108.7, 108.8 and Chapter 110 before submitting your records.
  2. Do not send original patient records.
  3. Records must be copied on one side of the paper (not both sides).
