Licensure by Credentials

General Qualifications for Licensure

  1. Be at least 21 years of age.
  2. Be of good moral character and professional fitness, which is demonstrated by patterns of personal, academic and occupational behaviors, including final or pending disciplinary action on any occupational license in any jurisdiction, which, in the judgment of the TSBDE, indicate honesty, accountability, trustworthiness, reliability, integrity, and ability.
  3. Pay all required fees. There is no application fee for military active duty, retirees, military active duty spouses.
  4. Submit an application for licensure.
  5. Provide information and documents as requested by the TSBDE.

Licensure by Credentials Requirements

Applicants must meet the following requirements:

  1. Graduation from dental school accredited by the Commission on Dental Accreditation (CODA) of the American Dental Association (ADA).  
  2. Successful completion of National Boards Parts I and II.
  3. Successful completion of state or regional general dentistry clinical exam.
  4. Have practiced dentistry:
    1. for a minimum of three (3) of the five (5) years immediately preceding application; or
    2. as a dental educator for the five (5) years immediately preceding application to Texas.
  5. Is currently licensed as a dentist in good standing in another state, District of Columbia, or a territory of the United States.
  6. Have no disciplinary actions or felony convictions.
  7. Complete a Level II Background check completed by the Professional Background Information Service (PBIS).  No Fingerprint Background Check is conducted when applying for Licensure by Credentials.

     Note: If you held a Texas Dental License that has been canceled, contact the TSBDE before you complete the PBIS Background Check process.
  8. Complete the TSBDE Jurisprudence Assessment within 12 months immediately prior to application.  (Fee Required)
  9. Complete 12 hours of Continuing Education (CE) taken within the preceding 12 months.
  10. Hold a current BLS CPR certification.​

Application Process

The following documents must be mailed to the TSBDE:

  1. Submit a fully completed Licensure by Credentials Application.  
  2. Application fee.  There is no application fee for military active duty, retirees, military active duty spouses.
  3. 2x2 passport photo
  4. Copy of driver’s license, naturalization papers, or passport.  
  5. Proof of current Basic Life Support CPR certification
  6. Proof of completion of 12 hours of Continuing Education in accordance with TSBDE Rule 104.2
  7. Letter issued by the PBIS stating your Level II Background Check for credentialing has been completed and sent to the Texas State Board of Dental Examiners.
  8. Certificate of completion of the TSBDE Jurisprudence Assessment taken within one year prior to application submission.
  9. National Practitioner Data Bank (NPDB) Self-Query.  All applicants are required to complete a self-query of the NPDB.  The report results must remain in the original sealed envelope and be attached to your application to the TSBDE.  NPDB self-query reports are valid for 30 days. NPDB charges a minimal fee to furnish this information.   10. Military Change of Station Orders or Military Identification Card or document showing proof of Honorable or General Discharge for military active duty, retirees or military spouses.

2-Step Application and Payment Process

Step 1.  Apply for your License By Mail.  

All applicants must mail a copy of their application and required documents to the TSBDE.

Step 2.  Paying to Activate Your License.  

  1. The status of your license will be Expired after it is first issued.
  2. You will receive a letter in the mail with instructions for paying your pro-rated Initial Renewal Fee online.  This fee will activate your license and change the status to Active.  This letter will authorize you to practice for 30 days.  Once you pay your fee online you should receive your new license in the mail approximately 7-10 days later.  
    Exception:  Active duty military, veterans and active duty military spouses are not required to pay to activate their license.  The license will be issued and mailed to the applicant after the application has been approved.  
  3. Your first license may be valid anywhere between 6 months to 17 months.  You will receive a reminder post card approximately 45 days prior to your license expiration date.

Additional Information

  • Anesthesia Permits.  Dentists must be issued a Texas Dental License before an Application for an Anesthesia Permit can be processed.
  • Change of Address.  Licensees are required to notify the TSBDE within 60 days of a change in address.
  • Continuing Education.  New licensees are exempt from completing CE during their first licensure period.  After the first license is renewed licensees should begin collecting CE.  CE must be completed in accordance with TSBDE Rule 104.2.
  • CPR Requirement.  CPR Courses must include a hands-on demonstration of skills by the applicant on a manikin and a written assessment of skills.
