Licensure by Examination

General Qualifications

  1. Dental hygienists must be at least 18 years of age. 
  2. Be of good moral character and professional fitness, which is demonstrated by patterns of personal, academic and occupational behaviors, including final or pending disciplinary action on any occupational license in any jurisdiction, which, in the judgment of the TSBDE, indicate honesty, accountability, trustworthiness, reliability, integrity, and ability.
  3. Pay all required fees.  (Fingerprint Criminal Background Check and Jurisprudence Assessment)
  4. Submit an application for licensure.
  5. Provide information and documents as requested by the TSBDE.

Application and Supporting Document Requirements

All other applicants must mail the following documents to the TSBDE:

  1. Submit a fully completed application. Submitting an incomplete application will delay your application process. 
  2. Application Fee or Online Payment Summary.   Military Active Duty, Veterans and Military Active Duty Spouses do not pay an application fee.
  3. A 2x2 passport photo. 
  4. Copy of driver’s license, naturalization papers, or passport.
  5. Copy of current Basic Life Support CPR certification.
  6. Official school transcript showing proof of graduation from a Commission on Dental Accreditation-accredited (CODA) dental hygiene school.  Transcripts must remain in the original sealed envelope.
  7. Proof of completion of Dental Hygiene National Boards.  Score reports must remain in the original sealed envelope.
  8. Proof of completion of a dental hygiene clinical examination administered by a regional clinical examining board dated within 5 years from the date of examination.  Score reports must remain in the original sealed envelope. Click here for a list of regional clinical examining boards recognized by the TSBDE.   If you took the WREB, CITA or CDCA exam since 2016, you do not need to send in a sealed score report.  You can send in a copy of your report and TSBDE staff will verify your score.  TSBDE staff has access to current results from WREB, CITA and CDCA.
  9. Proof of completion of TSBDE Jurisprudence Assessment within one year immediately prior to application.  
  10. Receipt showing proof you have completed a fingerprint criminal background check.  (  Fingerprint Criminal Record Check results are valid for 6 months from the date they were taken.
  11. National Practitioner Data Bank (NPDB) Self-Query.  Report results must remain in the original sealed envelope.  NPDB self-query reports are valid for 60 days.  Contact the NPDB at telephone number (800) 767-6732 or website
  12. American Association of Dental Boards (AADB) Clearinghouse Self-Query. Report results must remain in the original sealed envelope.   Download the  AADB Self-Query form from
  13. For applicants who have held a license in another state/jurisdiction: A verification of licensure with imprint of state seal issued from a state board of dentistry in which the applicant has ever held a license to practice dentistry or dental hygiene.   A copy of the license alone is not acceptable.
  14. Military Active Duty, Veterans or Military Active Duty Spouses must provide either a copy of Military Change of Station Orders, Identification Card or a document showing proof of Honorable or General Discharge.

Additional Information

  1. Change of Address.  Licensees are required to notify the TSBDE within 60 days of a change in address.
  2. Continuing Education.  New licensees are exempt from completing CE during their first licensure period.  After the first license is renewed licensees should begin collecting CE.
  3. CPR Requirement.  CPR Courses must include a hands-on demonstration of skills by the applicant on a manikin and a written assessment of skills.
  4. Foreign Language Documents.  All documents issued in a foreign language must be translated to English.

2-Step Application and Payment Process

Step 1Apply for your license by Mail or Online

All applicants must mail a copy of their paper application or online application summary and required documents to the TSBDE.

Step 2Paying to Activate Your License

  1. The status of your license will be Expired after it is first issued. 
  2. You will receive a letter in the mail with instructions for paying your pro-rated Initial Renewal Fee online.  This fee will activate your license and change the status to Active.  This letter will authorize you to practice for 30 days.  Once you pay your fee online you should receive your new license in the mail approximately 7-10 days later. 
  3. Your first license may be valid anywhere between 6 months to 17 months.  You will receive a reminder post card approximately 45 days prior to your license expiration date.
