Pit and Fissure Sealant Certification

  1. The Pit and Fissure Sealant Certificate is an optional certificate for Dental Assistants.  
  2. A Pit and Fissure Sealant Certificate issued by another state regulatory agency will not transfer to Texas.

Application and Required Documentation

The following must be mailed to the TSBDE:

  1. Complete the Pit and Fissure Sealant Certificate Application.
  2. Copy of your National Practitioner Data Bank (NPDB) Self-Query Report.  All applicants are required to complete a self-query to the NPDB.  The report results must remain in the original sealed envelope and be attached to your Pit and Fissure Sealant Application to the TSBDE.  NPDB self-query reports are valid for 30 days. NPDB charges a minimal fee to furnish this information.   3. Copy of your American Association of Dental Boards (AADB) Clearinghouse Self-Query Report.  Applicants are required to complete a self-query to the AADB Clearinghouse.  The report results must remain in the original sealed envelope and be attached to your Pit and Fissure Sealant Application to the TSBDE.   AADB charges a minimal fee to furnish this information.  The AADB self-query form can be downloaded from the AADB website at http://dentalboards.org/wp-content/uploads/2016/08/AADBSelfQueryForm.pdf.  The AADB self-query form should be mailed to:  American Association of Dental Boards, 211 East Chicago Avenue, Suite 760, Chicago, IL 60611.  The AADB telephone number is (312) 440-7464.
    (Important Note:  If you are using one envelope to mail more than one optional certificate application to the TSBDE you only need to mail one copy of your NPDB and AADB Self-Query Reports with your applications.  Optional Certificates Issued by the TSBDE include:  Pit and Fissure Sealants, Coronal Polishing, Nitrous Oxide Monitoring, Registered Dental Assistant (X-Ray) Certificate).
  4. Pay required application fee. Military Active Duty, Veterans, and Military Active Duty Spouses are not required to pay an application fee.
  5. A letter, preferably on letterhead, signed by a current or former employer showing proof that you have at least two (2) total years of Dental Assistant work experience. The letter must identify the dental professional you are/were employed with.
  6. Copy of Basic Life Support CPR Card. (Copied on one side of a full-size sheet of paper.) CPR training must have a hands-on demonstration of skills on a manikin and a written assessment of skills.
  7. Copy of your Social Security Card. (Copied on one side of a full-size piece of paper)
  8. Copy of your Certificate of Completion showing successful completion of a Pit and Fissure Sealant Course approved by the TSBDE.
  9. Military Active Duty, Veterans, and Military Active Duty Spouses are required to provide a copy of their Military Orders or Military I.D. Card.

2-Step Application and Payment Process

Step 1. Apply by mail.

All applicants must mail a copy of their application and required documents to the TSBDE.

Step 2. Paying to Activate Your License.

  1. The status of your license will be Expired after it is first issued.
  2. You will receive a letter in the mail with instructions for paying your pro-rated Initial Renewal Fee online. This fee will activate your license and change the status to Active. This letter will authorize you to practice for 30 days. Once you pay your fee online you should receive your new license in the mail approximately 7-10 days later.
    Exception: Active duty military, veterans and active duty military spouses are not required to pay to activate their license. The license will be issued and mailed to the applicant after the application has been approved.
  3. Your first license may be valid anywhere between 6 months to 17 months. You will receive a reminder post card approximately 45 days prior to your license expiration date.

Renewal of Certificate

The Pit and Fissure Sealant Certificate must be renewed annually within 45 days prior to the expiration date of December 31st.

Continuing Education Requirement

Dental Assistants who hold a Pit and Fissure Sealant Certificate are required to complete Continuing Education annually.
